Portsmouth, VA, USA1. Site of Service. The Contractor shall provide personnel for services in the Primary Care Clinic for Naval Medical Center, Portsmouth, VA and its Branch Clinics identified in Section B.
2. Labor categories. The contractor shall provide personnel from the following labor category:
Family Medicine Physician (Coverage)
3. Qualifications. The healthcare worker (HCW) shall possess and maintain the minimum qualifications stated in Section C of the basic contract. In addition to the qualifications in Section C of the basic contract, the HCW shall:
3.1. Possess board certification in Family Medicine as determined by the American Board of Family Medicine.
3.2. Possess a minimum of one year experience within the preceding 2 years.
3.3. Obtain Advanced Cardiac Life Support (ACLS) certification prior to contract start and maintain according to ACLS guidelines.
3.4. Provide two letters of recommendation written within the last two years attesting to the HCW’s clinical skills. At least one letter must be from a physician who has personally observed and evaluated the HCW and who can attest to the HCW’s clinical skills in the care of patients. The other letter may be from a clinic or hospital administrator or physician who has personal knowledge of the HCW’s patient care skills in a similar clinical environment. Reference letters shall attest to the HCW’s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference. Letters of recommendation must have been written within the last 2 years.
3.5. Possess and maintain a current Drug Enforcement Agency (DEA) registration number (certificate) to prescribe controlled substances as listed in 21 C.F.R. 1308.
